The ability to achieve and sustain erections requires the following:A healthy nervous system that conducts nerve impulses from the brain, spinal column, and the penisHealthy arteries in and near the corpora cavernosa of the penis so that when stimulated there is an increase in blood flow to the penisHealthy smooth muscle and fibrous tissue within the corpora cavernosa so that it can fill with bloodAn adequate amount of nitric oxide (NO) in the penis to help with the stimulation of blood flow into the penisNormal functioning of the tunica albuginea, the layer of tissue surrounding the corpora cavernosa and responsible for the compression of the veins to keep blood in the penisAppropriate psychosocial interactions to enhance sexual stimulation/arousal and decrease anxiety/stress

The penis contains three cylinders, the two corpora cavernosa, which are on the top of the penis (see figure 1 below). These two cylinders are involved in erections. The third cylinder contains the urethra, the tube that the urine and ejaculate passes through, runs along the underside of the penis. The corpus spongiosum surrounds the urethra. Spongy tissue that has muscles, fibrous tissues, veins, and arteries within it makes up the corpora cavernosa. The inside of the corpora cavernosa is like a sponge, with potential spaces that can fill with blood and distend (known as sinusoids). A layer of tissue that is like Saran Wrap, called the tunica albuginea, surrounds the corpora. Veins located just under the tunica albuginea drain blood out of the penis. Understanding the signs and causes of ED is the first step toward achieving a better sex life.
Pelvic area muscles are the ones that help in increasing the blood flow to the groin and helps in maintaining erections. Strengthening the muscles with pelvic floor exercises can actually improve sexual functioning like erections, ejaculations, and orgasm.
When the aforementioned strategies prove insufficient to address ED symptoms, medical therapies are warranted. The simplest treatments include use of a class of oral medications known as phosphodiesterase type 5 inhibitors (PDE5I, e.g. sildenafil, tadalafil, vardenafil, and avanafil). These drugs enhance the effects of nitric oxide, a natural molecule within the body that relaxes the smooth muscle of the penis, to enable increased blood to flow and result in erection. These medications are generally very safe and reliable. Men who use nitrates (found in certain heart medications and in some recreational drugs) should not use PDE5I. Optimal use of PDE5I requires advice on timing and proper use of these drugs for maximal effect.

Online medicines are not always regulated and the ingredients in them can vary from one pack to another. They can cause unpleasant side effects, or may not be suitable for you.
Another way to increase the flexibility of your pelvis and restore sexual energy is by stretching your hip joints specifically. To do this, go into a plank and push your hips upwards and then downwards in an alternate motion. This will also strengthen your lower back and tone your thigh muscles. Your core is also made stronger when you maintain this position for some time. The arms and shoulders, moreover, are strengthened when they carry the weight of your upper body. Go into a plank position. Place your palms firmly on the floor in line with your shoulders. Extend your legs at the back. Keep your elbows and knees straight. Bring your body in one straight line from head to toe. Raise your hips so that your body is in the shape of a mountain. Lower your hips and bring it as close to the floor as possible without touching it. Repeat in a seamless motion. Bridge pose
The recreational drug ‘poppers’ should not be used with any erectile dysfunction medications as this could result in a potentially fatal drop in blood pressure.

The lab testing obtained for the evaluation of erectile dysfunction may vary with the information obtained on the health history, physical examination, and recent lab testing. A testosterone level is not necessary in all men; however, a physician will order labs to determine a patient's testosterone level if other signs and symptoms of hypogonadism (low testosterone) such as decreased libido, loss of body hair, muscle loss, breast enlargement, osteoporosis, infertility, and decreased penile/testicular size are present.
While ED medication works for most patients, there are other treatment options. These include external vacuum devices that draw blood into the penis and injections that men administer themselves to dilate the arteries of the penis and allow blood to flow in. Another option is penile implants, which Dr. Walker calls the “gold standard” for men who cannot tolerate the drugs and for whom other treatments fail. Flexible cylinders are embedded in the penis. The man pumps fluid into them from a reservoir when he wants to perform sexually.
