Hemoglobin A1c or HbA1c is a protein on the surface of red blood cells. The HbA1c test is used to monitor blood sugar levels in people with type 1 and type 2 diabetes over time. Normal HbA1c levels are 6% or less. HbA1c levels can be affected by insulin use, fasting, glucose intake (oral or IV), or a combination of these and other factors. High hemoglobin A1c levels in the blood increases the risk of microvascular complications, for example, diabetic neuropathy, eye, and kidney disease.
Erectile dysfunction can result from a number of factors such as medical conditions, psychological conditions, certain medical treatments, injuries and drug and alcohol use. While most cases of erectile dysfunction are caused by physical issues, sometimes a combination of physical and psychological issues can contribute to developing the condition.
Men with a rare heart condition known as long QT syndrome should not take vardenafil since this may lead to abnormal heart rhythms. The QT interval is the time it takes for the heart's muscle to recover after it has contracted and is measured on an electrocardiogram (EKG). In addition, vardenafil is not recommended for men taking medications that can affect the QT interval such as quinidine, procainamide, amiodarone, and sotalol.

ED is rarely caused by severe pelvic trauma from major accidents in younger patients. Revascularization is an operation to bring new or additional blood vessels to the penis to improve the circulation that may have been severed by the initial trauma. This is a very rare type of treatment and is not recommended for the typical patient. After identification of a psychogenic source for ED, we will refer patients to a therapist with expertise in treating sexual dysfunction. We often recommend low dose medications for ED in conjunction with the therapist’s recommendations.

Treatment options for ED have evolved considerably over the past decade to encompass psychological counseling; oral, topical, intraurethral, and intracavernosal vasoactive therapy; oral therapies with other or unknown mechanisms; hormone replacement; vacuum constriction devices; and surgery, including vascular bypass procedures and penile implants. The goal of treatment is to restore satisfactory erections with minimal adverse effects. Men have demonstrated a strong preference for oral treatments even if they have low efficacy.
No matter what type of ED men suffer with, the first step is going to the doctor—the earlier, the better. This can be especially difficult for younger men.

Penile erection is caused by the engorgement of the penis with blood. This engorgement occurs when the blood vessels delivering blood to the penis increase the delivery of blood and the blood vessels carrying blood away from the penis decrease the removal of blood. Under normal conditions, sexual stimulation leads to the production and release of nitric oxide in the penis. Nitric oxide then activates the enzyme, guanylate cyclase, which causes the production of cyclic guanosine monophosphate (cGMP). It is the cGMP that is primarily responsible for the erection by affecting the amount of blood that the blood vessels deliver and remove from the penis.
