ED is rarely caused by severe pelvic trauma from major accidents in younger patients. Revascularization is an operation to bring new or additional blood vessels to the penis to improve the circulation that may have been severed by the initial trauma. This is a very rare type of treatment and is not recommended for the typical patient. After identification of a psychogenic source for ED, we will refer patients to a therapist with expertise in treating sexual dysfunction. We often recommend low dose medications for ED in conjunction with the therapist’s recommendations.
Men who want to treat their ED have another option. The GAINSWave® treatment is a safe, non-surgical therapy that can restore sexual function and help to prevent erectile dysfunction in men who don’t have ED.

Because ED and atherosclerosis are often linked, it is important to check cholesterol levels. Similarly, the doctor may order tests for blood sugar (glucose) levels to check if diabetes is a factor. A blood test to measure total serum testosterone is usually done. This test should be done in the morning.
Aging and ED can slow men's sexual health, but pleasure is not only possible, it can also improve quality of life in men 60 and older.

The penis is composed of three cylinders, two on the top and one on the underside of the penis. The top two cylinders are involved in the erectile process. The urethra, the tube that urine and semen pass through, is on the underside of the penis. The top two penile cylinders, the corpora cavernosa, are composed of tissue that is analogous to a sponge, containing spaces that can fill with blood and expand. These two cylinders are surrounding by a strong layer of tissue, like Saran wrap, the tunica albuginea. For an erection to occur, there must be properly functioning nerves, arteries, veins, and normal penile tissues. Share Your Story
These drugs are generally safe and effective for most men. They may not be appropriate for men with certain health conditions, such as severe heart disease, heart failure, uncontrolled high blood pressure, diabetes, or a history of stroke or heart attack. Men who take nitrate drugs cannot use PDE5 inhibitors. PDE5 inhibitors can also interact with other medications. Talk to your doctor about whether PDE5 inhibitor drugs are a safe choice for you.
Older men are more likely to take medication for health problems, which could influence their risk.
While performing Kegels, men sometimes forget some minor details that should be avoided to minimize risks or mishappenings. Here are some of those:
Certain street drugs such as "poppers" also can cause serious problems if taken with PDE5i medications. These poppers are often types of nitrates and can cause severe drops in blood pressure. Ecstasy is another street drug that may increase sexual desire but interferes with performance. This has prompted some men to combine ecstasy with PDE5i medications. This mixture (a combination sometimes called "sextasy") can improve erection ability but also causes severe headache and priapism. (Priapism is an abnormally prolonged erection that becomes extremely painful and may result in permanent damage to the erection mechanism.) There are also potentially dangerous effects to your heart from mixing PDE5i medications with various other street drugs.

First-line treatment is typically with the use of phosphodiesterase inhibitors, such as sildenafil (also known as Viagra). In the UK, Viagra is now available without a prescription, and can be obtained over the counter from pharmacists, once suitability has been assessed. A vacuum device, or pump, can also be useful especially if medications are not suitable. Vacuum pumps work by encouraging blood flow to the penis, resulting in an erection.
It is important to distinguish between a normal decline of testosterone levels and hypogonadism.
Erectile dysfunction can be a difficult problem to discuss with your partner or even your doctor. However, it needn't be because erectile dysfunction, which causes sexual performance issues for men, is a very common and highly treatable condition.
