Doctors have not tested the PDE5 inhibitors in patients with retinitis pigmentosa, an eye condition that affects the retina and can cause blindness, and so caution is recommended if you have this health condition.
It has been estimated that impotence affects 140 million men worldwide. Over half of all men with impotence are thought to have some physical (medical) cause. The remainder are believed to have psychogenic causes of impotence. Medical causes of impotence include diabetes and circulatory, neurological, or urological conditions.

Pelvic floor muscles can be identified by attempting to stop the flow of urine mid-stream. These are the muscles that will be used. Start with an empty bladder and tighten the pelvic floor muscles for 5 seconds, then relax for 5 seconds. Attempt to do 5 repetitions (reps) the first day. Work up to 10 seconds at a time. Aim for 3 sets of 10 reps daily. Avoid tightening muscles in the abdomen, thighs, or buttocks and avoid holding your breath. All four of the PDE5 inhibitors (Viagra, Cialis, Levitra, and Stendra) are approved by the Food and Drug Administration for on-demand use for erectile dysfunction.
A meta-analysis of 36 744 men with ED in 12 prospective cohort studies found that the presence of ED significantly increased the risk of CVD, CAD, stroke and all-cause mortality, and the presence of ED was an independent risk factor for CVD. Ponholzer et al found that men with moderate to severe ED had a 65% increased relative risk for developing symptomatic CAD compared with men who did not have ED.26
Vacuum erection devices, also called vacuum constriction devices, are another option for men with ED. They are available without prescription and have a high success rate, but are cumbersome and may be difficult to use for some men. They typically work as follows:
One of the most important causes for ED is vascular disease; examples include diabetes (Types I and II), high blood-pressure, high cholesterol/lipids, tobacco use, obesity, and lack of exercise. All of these conditions can lead to impaired response of the blood vessels responsible for controlling penile erection. There is evidence that men who have vascular disease and make healthy lifestyle changes (e.g. increasing exercise, losing weight, quitting smoking) may experience improvement in erectile function. It is important to remember that whatever is good for the heart is also good for the penis.
Many people think that penis-in-vagina sex or sex with penetration are the only ways to have sex. But, most couples use touching and caressing as part of their routine. When couples can’t have intercourse because of erection problems, many give up on sex, but others continue to have sex in other ways. Think about trying something new.
Medications: Medicines that cause a strong penile erection are successful in treating erectile dysfunction in most men. These medications work by increasing the amount of blood flow to the penis by increasing nitric oxide levels in the blood. Examples include:
Patients may take tadalafil as needed as with sildenafil, vardenafil, and avanafil or once a day. It is the only ED oral medication that patients can take on a daily basis.
A plastic cylinder, in which the individual places his penisA pump, which draws air out of the cylinderAn elastic band, which is placed around the base of the penis, to maintain the erection after the cylinder is removed and during intercourse by preventing blood from flowing back into the body (see figure 2).The vacuum device may be operated by hand or be battery operated.

In the majority of men who suffer from erectile dysfunction, the cause is likely poor circulation. Poor circulation may be caused by numerous different health issues. In order to diagnose erectile dysfunction, your medical professional will begin by asking you questions about your vascular and heart health along with your erection issues. Your doctor may also perform a physical examination or may refer you to a urologist for lab tests.
Other medications can also contribute to erectile dysfunction. These include some drugs taken to treat depression, other mental health conditions and high blood pressure, as well as opioid painkillers. Many recreational drugs cause it too.
Despite conflicting findings in clinical studies, men should monitor the effects of alcohol use on erectile function. Alcohol can lower testosterone levels, decrease blood flow into the penis, and cause fatigue. At least in some men, alcohol can exacerbate symptoms of E.D.
